Privacy Release: The following release authorization is for volunteers 18 years of age and older. I certify that all information in this application is true and complete. I understand that any false information or omission may disqualify me from further consideration for volunteer service and may result in my dismissal if discovered at a later date. I understand that Hope & Home requires certain information both personal and professional from me to evaluate my qualifications and consider me for volunteer service. I understand that in consideration of my application, a background investigation may be conducted. I understand that any information requested is for the sole purpose of gathering information accurately for use in the above-mentioned volunteering and background check. I have read and understand the above, and by typing my name in the box below, consent to these statements.
Physical Waiver and Photography Permission: In consideration of the acceptance of this entry, I waive all claims for myself, my spouse and my heirs against Hope & Home for any injury or illness, which may result directly or indirectly from my participation. I further state that I/we am/are in the proper physical condition to participate. In addition, I also give Hope & Home permission to use photographs of me and my party taken at Hope & Home and associated events in future event promotion. Typing my name in the box below serves as acceptance of the above statement.
