Family Care Coordinator – Colorado Springs

The Family Care Coordinator supports the clinical and programmatic activities of Hope & Home through their role in the Home Team.  Hope & Home emphasizes a supportive and intentional experience for its families caring for children in foster care. Family Care Coordinators facilitate communication and interaction amongst the Home Team in an effort to centralize support for families. Their primary responsibilities include ensuring family and children files are in compliance with rules and regulations.

Primary Responsibilities Include:

  • Case Aid:
    • Manage a caseload of families as a team with three to four Home Supervisors and Hope Workers
    • Answer questions for families, daily or as needed, and coordinate the Home Team to meet family needs
    • Prepare audits and checklists for home visits, emailing families any paperwork needs
    • Manage a constant stream and large volume of incoming, confidential paperwork from numerous sources
    • Manage requisite dates and audits of files communicating needs with families and Home Supervisors
    • Work with the Home Team to ensure family and staff compliance with Volume 7, Love to Nurture, and organization rules, regulations, and requirements
  • Reporting and Paperwork:
    • Work with Home Supervisors, Compliance Managers, and Background Check and Contract Specialist and foster families to ensure files are 100% compliant
    • Work closely with staff to ensure all family and child records are complete in accordance with all rules and regulations
    • Prepare 90-day documents for recertification, complete quarterly reports, update health passports, and complete yearly audits
    • Request necessary information and documentation from County Caseworkers and Home Supervisors as required
    • Track ongoing training requirements for families and provide monthly updates
  • Other:
    • Participate in on-going training
      Participate in Hope & Home activities and events as specified by supervisor
      All other duties as assigned
  • Events           
    • Participate in Hope & Home events for families and children as required and instructed by manager and Hope & Home leadership (generally 3-4 events per year)
    • Work at the annual Hope & Home Fashion Show fundraising event
  • Other:
    • Participate in weekly group supervision and individual supervision as scheduled with manager
    • Participate in required training annually, as specified by manager and director of education
    • Complete on-call duties as assigned by rotation
    • All other duties as assigned

Pay and Benefits: 

  • $17/hour.
  • Full time.
  • Paid time off accruing immediately upon start of work and useable once accrued.

  • Medical, dental, and vision insurance (eligible the first of the month following 60 full days of employment).
  • Enrollment in 401K after working at least one year and 1,000 hours, beginning the quarter following eligibility (must be 21 years of age to participate).
  • Company paid life insurance, with employee option to add additional coverage and long-term disability coverage (if working 20 or more hours per week).

Please Note: Hope & Home’s benefit package is contributory; that is, you are responsible for a portion of the premium for your benefits. A portion of the premium, up to a maximum per month, is contributed by Hope & Home. Your contributory cost is deducted from your paycheck.
