I understand that in consideration of my application, a background investigation may be conducted. I understand that any information requested is for the sole purpose of gathering information accurately for use in the above-mentioned volunteering and background check. Physical Waiver and Photography Permission: In consideration of the acceptance of this entry, I waive all claims for myself, my spouse and my heirs against Hope & Home for any injury or illness, which may result directly or indirectly from my participation. I further state that I/we am/are in the proper physical condition to participate. In addition, I also give Hope & Home permission to use photographs of me and my party taken at Hope & Home and associated events in future event promotion.
