Application

Congratulations and welcome! We’re grateful that you’ve decided to take the first step towards welcoming children in foster care into your home. The journey to becoming a foster parent is one that we hope you’ll find fulfilling, encouraging, and meaningful. Below are the “first steps” in this process.

Colorado Application to Care for Children and Youth

This is a standardized application designed to gather all the appropriate information in order to officially approve you to become a foster parent. The application can be lengthy and depending on your family history may take 30 minutes to 1 hour to complete. You are welcome to begin the application and come back to it, using the instructions presented on the form.

Please complete this application prior to your Admissions Meeting and email the completed form to your Licensing Specialist. (TIP! The sooner the better, if completed more than 24 hours before your meeting your Licensing Specialist can pre-fill some additional documents you’ll complete during the Admissions Meeting.)

IMPORTANT! YOU MUST FIRST DOWNLOAD THIS PDF IN ORDER TO SAVE THE FORM FIELDS YOU COMPLETE. If you complete the form in your internet window and then download and save it, the fields will not save when downloaded.

Letter of Intent and Family Photo

The letter of intent helps us get to know you better. Take some time to think through your foster care goals and expectations, as well as your family strengths and how you can help children in foster care.

Including a family photo helps staff along the way put a face to the name. Please include all members of your immediate family (furry friends are encouraged to be included too!).

Document Uploads

Included in your Licensing Roadmap, is a list of documents that you’ll need to either upload digitally or bring hard copies of to your Admissions Meeting. We’ve found that uploading digital copies is easier and faster for families, and ensures that your hard copies of these valuable documents never have to leave where you store them for safe keeping! Use this same link multiple times for each document upload.

If you have any questions, please be sure to contact your Licensing Specialist!
