Step 1

Colorado Application to Care for Children and Youth

This is a standardized application designed to gather all necessary information to officially approve you to become a foster parent. The application can be lengthy and depending on your family history may take 30-60 minutes to complete.

Please complete this application prior to your Admissions Meeting and email the completed form to your Licensing Specialist.

*TIP! If completed more than 24 hours before your meeting, your Licensing Specialist can pre-fill some additional documents that will expedite your Admissions Meeting.

Important! You must first download this PDF in order to save the form fields you complete. If you complete the form in your internet window and then download and save it, the fields will not save when downloaded.

Letter of Intent and Family Photo

The letter of intent helps us get to know you better. Take some time to think through your foster care goals and expectations, as well as your family strengths and what you can offer a child(ren) in foster care.

Including a family photo helps staff put a face to the name. Please include all those who reside in your home–furry friends included!
