Required:
- CPR and First Aid Card(s)
- Floor Plan
- Completed Physical Form(s)
If Applicable:
- Current Flu/Pertussis Immunization Records for each adult (if open for placement ages 0-2)
- Immunization Records for each child — must meet the CDC recommendation for age
- Well water test results
